Wako City subsidizes medical expenses for your child/children!

Wako City subsidizes medical expenses for children to increase child health and enhance child welfare. If your child/children receive medical treatment for illness or injury, covered by health insurance, Wako City will subsidize the self-pay burden of medical expenses. Pre-registration is required to receive the subsidies. Please bring the health insurance card (or certificate) and apply for at Children’s Welfare Division. Applications are also accepted by mail.

【Available period】
 From the date your child moved into Wako City, and up to March 31, after he/she has reached the age of 6.
 From the date of birth of your child, and up to March 31, after he/she has reached the age of 6.
 If your child is to move out from Wako City, he/she is eligible before the date of moving out.

【Note】
 Your child will be eligible to receive the subsidies from the date of birth or the date of moving into Wako City, only if your child is registered within 15days from the date of birth or the date of moving into Wako City. If your child is registered after 15 days from the date of birth or the date of moving into Wako City, your child is eligible to receive the subsidies from the date of application.

 Health insurance card (or certificate) including your child’s name, or parent’s (or guardian’s) health insurance card (or certificate) on which your child’s name is expected to add.
 Proof of the applicant’s bank account, such as a bank passbook or an ATM card. (Guardian’s 〔main insured person’s〕bank account only.)

 A photocopy of health insurance card (or certificate) including your child’s name, or that of parent’s (or guardian’s) health insurance card (or certificate) on which your child’s name is expected to add.
          ※ We can pay only into the guardian’s (main insured person’s) bank account.
          ※ Arrival date of your application is to be the accepted date.
