Certificate of residence

  • The purpose of Resident registry (Certificate of residence) is to prove your resident status. It is necessary when registering electoral register, entering elementary/middle school, enrolling National Health Insurance and National Pension.
  • Since July 9th, 2012,‘Certificate of residence’has been issued to foreign residents living in Japan(there are some not qualified).

  • Notification such as ‘Notification of moving in’must be made by designated date (within 14days of moving in).

  • Copy of ‘Certificate of residence’is issued at Registration and Residents Division and Branch Offices.

When moved in Wako-City from other cities

Make notification along with former municipality-issued‘Certificate of movingout’at Registration and Residents Division within 14days of living in Wako-City.

Report by principal or householder.
Letter of attorney is needed when non-family member makes notification.

  • Health care certificate for people aged 75 and older, in which described grade of imposition (those moved from other prefectures)

When moved in Wako-City from overseas

Make notification along with ‘special permanent resident certificate’or‘residentcard’at Registration and Residents Division within 14days of living in Wako-City.If the householder in the same household is foreign resident, official document that verifies his/her relationship to notifier is required (Japanese translation is needed if the document is written in foreign language).

Before moving, report the date of moving and new address to Registration and

Residents Division, and request issuance of ‘Certificate of moving out.’
After moved to new address, report to municipality you move in within 14days of arriving..

Submitting Notification of moving outby mail.

Application form can be letter paper. In this case, note Request for ‘Certificate of moving out’in the beginning of writing, and fill out following information.

  • Name of notifier and put his/her seal.
  • Date of moving (the date you start living in new address).
  • New address and householder of the address.
  • Present address and householder of the address.
  • Address of legal domicile and name of the head of the address.
  • Name, dates of birth, sex and relationship of all members who move out.
  • Telephone number for daytime contact (8:30a.m-5:15p.m)

Enclose (1) paper filled out information above, (2) Stamped envelope (we send you Certificate of moving out’) (3) Copy of identification such as driver’s license. And mail to Registration and Residents Division.  Address   see

When move to overseas
